Employment Status: Full-Time, Salaried, Commensurate with experience. 
 
 
Minimal Qualifications/Experience/ Skills:

Bachelor's degree in social services field. Case management experience required. AFC certified case manager preferred or abilit to become certified within 90 days of hire. Must be knowledgeable about HIV/AIDS. Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is required. Must have computer literacy and data entry skills. Strong organization and time management skills required as well as the ability to work independently. An automobile, valid Driver's License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.

Job Summary/Overview:

The Supportive Services Case Manager assists medically stable and adherent HIV positive clients in accessing support services including medical, social, community, legal and financial services. 
 
Responsibilities: 
  • Manages a caseload of 75-100 medically stable clients
  • Conducts initial intake assessments
  • Conducts routine health and social assessments
  • Administers individual service plans and assesses client progress toward goals
  • Routinely assesses strengths and barriers to maintaining stabilized health outcomes
  • Assists client in developing personal support systems
  • Provides advocacy, support, and linkages/referrals to community resources
  • Maintains client file and records all activity
  • Routinely enters all client data into client-level database
  • Prepares routine reports on activities of program
  • Develops and maintains relationships with community agencies
  • Completes the Case Management Competencies training with a passing grade as determined by the Training Policy
  • Attends at least 12 trainings durng the contract year to enhance skills/HIV knowledge
  • Attends monthly Team meetings and all other relevant program meetings as scheduled
  • Performs other duties as required or assigned
