RESPONSIBILITIES:
The Diabetes Care Coordinator collaborates with an interdisciplinary team of providers, administrators, health promoters and quality improvement specialists to support diabetic patients across Erie Family Health Center locations. This position identifies and coordinates additional resources for diabetic patients including health education, transportation, and completion of lab tests and referrals. The Diabetes Care Coordinator is responsible for implementing and coordinating group diabetes visits, educational sessions and specialty clinics.
This position manages data collection from diabetes group visits and provider patient panels for health center and grant reports. An ideal candidate would enjoy working as part of an interdisciplinary team to coordinate resources and improve health outcomes for diabetic patients.
REQUIREMENTS:
Minimum of BA required, Masters of Public Health or related field preferred. Experience with project coordination or program management, strong interpersonal and oral communication skills. Bilingual (Spanish and English). Experience working with diabetic patients and one or more years working in community health setting preferred. Demonstrated cultural competency to work in a low income, minority environment. Experience with data entry in various systems including Electronic Health Record System.
