LOCATION: CEDA Central – Chicago, IL
DEPARTMENT: Children and Family Services – Head Start (HS)
Description: Position functions to provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs in the area of Healthy Marriage and Economic Stability activities. Position seeks to enhance the quality of client management and satisfaction, the continuity of services and the cost effectiveness of the project through the integration of case management.
Duties:
Include, but are not limited to, coordinating social services/case management into client services by completing assessments of client needs; developing and maintaining case notes to track client services and progression towards goals and self-sufficiency; working with program participants to create career plans; identifying potential employers to develop employment opportunities for program participants; providing case management support to assist participants in eliminating barriers that may interfere with obtaining and/or retaining employment; assisting with coordinating employment-related activities/events; serving as a client advocate to enhance collaborative relationships with referral agencies to achieve goals outlined in program participants’ life improvement plans; preparing written reports outlining job development and placement activities; coordinating the provision of social services to clients’ families/significant others to enable them to deal with the impact of problems in their economic situation; maintaining contact with clients on a scheduled basis that is appropriate to their particular status and needs; maintaining outreach to agencies/organizations pertinent to client needs to receive supportive services; keeping abreast of employer satisfaction by contacting employers regularly to provide placement follow-up; and assisting with employment readiness workshops, conference and seminars.
Qualifications:
Bachelor’s degree, Human Services, Social Services; minimum 3 years of case management experience. Knowledge of community resources that serve targeted population. Computer proficient (Microsoft Word, Excel, PowerPoint). Bilingual (Eng/Span) interpretation and translation a plus.
Travel: High local travel; automobile required.
