JOB TITLE: FAMILY SERVICES SPECIALIST

Location: Ray Walker Head Start – Schaumburg, IL
Department: Children/Family Services
DESCRIPTION: Position is responsible for the overall case management of services as it relates to Social Services, Health, Nutrition and Mental Health, and Disability Services of the local area Head Start Program service center.
Duties: Include, but are not limited to:
· Implementing recruitment efforts for the Center including children with disabilities;
· Establishing and maintaining accurate documentation of age/income eligibility for children and pregnant women participating in the program;
· Providing case management services to families and children including:
Collaborating with families in identifying and defining goals as early as possible after enrollment and providing intervention, referrals and assistance for areas identified; assisting families to access community services that meet their needs as well as maintaining follow-up; collaborating with local health agencies to ensure all children are linked to a medical and dental home for ongoing and continuous service; providing immediate referrals, assistance and follow-up for crisis intervention needs;documenting and providing follow-up regarding children’s absences in the program including home visits as needed; maintaining ongoing contact and documentation with families regarding program services;
· Participating in staff/parent conferences by sharing developmental information files;
· Referring couples/parents to the Healthy Marriage and/or Economic Stability programs;
· Training staff and parents on the utilization of the Community Resources Book;
· Recruiting/assisting with the coordination of the local Health Services Advisory Committee;
· Participating in the collection of data for the local Community Assessment;
· Assisting with the provision of family literacy activities and various parent workshops;
· Participating in the monitoring system designed for CEDA Early/Head Start, ensuring follow-up on all issues found during technical assistance/monitoring visits.
Qualifications:
BA degree, Social Serv., Human Serv. or Early Childhood Education. 2 years exp. in a HS prg. or knowledge of local comm. resources. Ability to develop and maintain accurate recordkeeping systems, case notes and establish relationships with families/children from various socioeconomic backgrounds.
Travel: Throughout service delivery area. Automobile required.
