Job Title:Nurse Care Coordinator - Sickle Cell
Reports to: Project Manager Status: Part Time Open To:In-house, Public Qualifications/Experience/Skills:Licensed Practical Nurse, (Registered Nurse preferred) Minimum of three years experience within the health care field, preferably within sickle cell disease/case management arena. Excellent leadership, advocacy, communication, organizational, education and counseling, resource research, and planning skills. Job Summary/Overview:Provides overall coordination to promote comprehension and continuity of care for clients with sickle cell. Provides day to day implementation of the care coordination activities. Responsibilities: Demonstrate and apply knowledge of the philosophy/principles of comprehensive, community based, family-centered, developmentally appropriate, culturally sensitive care coordination services Facilitate family access to primary care providers, staff and resources Use a team approach to promote the identification of patients in practice with sickle cell diseases; add to registry and use to plan and monitor care Assess client and family needs and unmet needs, strengths and assets Initiate client/family contacts; create ongoing processes for clients/families to determine and request the level of care coordination they desire for themselves, child/youth or family member at any given point in time Build care relations among family and team; support the primary care-giving role of the family Coordinate and facilitate development and implementation of plan of care for clients using identified goals and outcomes with family involvement; use age appropriate transition timetables for interventions within care plans Serve as the contact point, advocate and informational resource for family and community partners/payers Research, find and link resources, services and supports with/for the client/family Educate, counsel, and support; provide developmentally appropriate anticipatory guidance; in a crisis, intervene or facilitate referrals appropriately Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow-up and integration of information into the care plan Coordinate inter-organizationally among client, family, primary care and involved agencies; facilitate wrap-around meetings or team conferences; offer outreach to community related to the population of sickle cell clients Work in collaboration with the Project Manager and Quality Officer to help to measure quality and to identify, test, refine and implement practice improvements.
