Reports to:Clinical Director of HIV Services
Core Responsibilities:
The Patient Health Advocate II meets with all newly enrolled patients living with HIV, to obtain first primary care visit and conduct intake. Orientates all new patients to available services. Make transportation arrangements for patients when needed. Coordinates patient services. Keep ADAP paperwork updated. Help to resolve patient problems or issues. Facilitates educational/life skills groups. Input client level data into RW CareWare.
Other Responsibilities:
1.Complete client paperwork including intake, nutritional assessment, initial adherence assessment, and ADAP forms.
2.Complete assigned reports, i.e., Surveillance
3.Maintain client level reporting system and perform data entry
4.Maintain charts for compliance with grants.
5.Network with HIV community to enhance program.
6.Participate in HIV community planning and service delivery workgroups.
7.Coordinate patient services, i.e., referrals for dental, mental health, etc.
8.Facilitate individual and group educational/enlightenment sessions (secures speakers, space, food and transportation for participants).
9.Monitor and oversee the access of care for new positives into CCHC primary care services
10.Provide adherence education for patients that are newly prescribed or have a change in Highly Active Anti-retroviral Therapy (HAART.
11.Collaborate with CCHCs Counseling and Testing Initiative.
12.Collaborate with CCHCs DIS on Partner Services for new clinic positives.
13.Inform and support patients in empowerment and advocacy opportunities.
14.Assist with the coordination, development and implementation of the Consumer Advisory Board.
15.Conduct annual focus groups
16.Conduct quarterly patient satisfaction surveys.
17.Maintain working relationships with HIV team members.
18.Attend all program related meetings

QUALIFICATIONS
High school graduate or G.E.D. equivalent with a minimum of five years of recent HIV prevention and/or care experience.
Must be a good listener, not shy about assessing HIV risk and providing education pertaining to that risk.
Excellent communication and organizational skills
Computer skills a must
