Description of Duties:

The Community Support Specialist will work within a team structure and provide direct care services to members served in a supportive housing environment. Services include: Monitor and promote the members daily activities including meals, medication, leisure and pre bedtime activities; Care Plan treatment and development and evaluation: linkage to services both in the community and within Thresholds; documentation of services provided; and work with other teammates with the goal of providing individual and group services.

Minimum Education required:  

Bachelor Degree Preferred; will consider years of experience in the mental health field
