Minimal Qualifications/Experience/Skills:
Bachelors degree plus two (2) years experience in social services (case management experience preferred). Individuals without a Bachelors degree, but have extensive case management experience may be considered. Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred. An automobile, valid Drivers License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.

Job Summary/Overview:
Under the direction of the Housing Program Manager, the Housing Case Manager assists supportive housing clients maintain their housing and increase their self-sufficiency though linkages to resources and the attainment of established goals.
Responsibilities:
Schedules and completes home visits on a routine basis
Administers individual service plans and assesses client progress toward goals
Routinely assesses strengths and barriers to increasing income, housing stability and increasing
self-sufficiency (including health, substance use and mental health issues)
Provides advocacy, support, and linkages/referrals to community resources
Conducts one-on-one life-skill training with clients
Maintains client file and records all activity
Prepares monthly/quarterly/annual reports on activities of program
Develops and maintains relationships with community agencies and landlords
Attends professional workshops and seminars to enhance skills
Attends monthly Housing Team meetings and all other relevant program meetings as scheduled
Performs other duties as required or assigned
