Nurse Case Manager – Stand Against Cancer (SAC)
SAC is a state-wide collaboration of Illinois faith-based organizations, healthcare facilities, and the American Cancer Society funded by the Illinois Department of Public Health to reduce racial and economic disparities in access to breast and cervical cancer screening and treatment. This mission is accomplished through outreach and education in high-risk communities, access to free or low cost screening exams, and coordination of care and services for women with abnormal screening results by means of case management. The goal of case management is to ensure that women enrolled in the program receive timely and appropriate re-screening, diagnostic, and treatment services. Overall SAC provides an enhanced, coordinated system of care for the uninsured women in the state of Illinois.
Qualified candidates will be licensed as a Nurse in the State of Illinois; Previous experience in women’s health or cancer programs preferred; Previous case management experience preferred; and Bilingual (English/Spanish) required.
